CREDIT CARD GUARANTEE OF PAYMENT AUTHORIZATION
AGREEMENT
I have reviewed the price list and service terms. I hereby authorize
Best Parking Lot Cleaning, Inc. to charge the credit card noted above including
fees, costs, and expenses associated in performing the services that I have
requested. I understand that charges will be made to this credit card account
as soon as services have been performed. I further agree that in the event that
my credit card becomes invalid, I will immediately provide a new valid credit
card to be charged an additional $35 fee along with any outstanding balances
owed.
